PreK 3 & 4yr programs: Physical.
Kindergarten: Physical, eye exam by optometrist or ophthalmologist;
2nd Grade: Dental exam.
6th Grade: Physical & dental exams.
FYI: Due to the recent outbreak of Whooping Cough around the state,
the Illinois Department of Public Health is requiring ALL 6th and 9th
graders this year to receive a booster on their TDap immunization.
These two grades were targeted because these students are required to
have a school physical, and can receive the booster at that time.
NEXT YEAR the IDPH is requiring ALL school age children to have this
NOTE: All students in grades 5, 7, and 8 that will be participating
in sports this year need to have a "sports"physical on file in the
office. If your student is a 6th grader - they will NOT need a
separate sports physical-their school physical covers that
requirement. BUT a "sports physical" does NOT fulfill the school
physical requirement. School physicals are much more in depth and
also include verification of immunizations.
